Life24 is quite literally a lifeline for many people. It enables patients who are being discharged from hospital, the elderly, and long-term condition/disability sufferers, to live with independence in their own home.
To this end we work closely with hospital discharge terms, commissioners of care packages, social services and GPs, who have a duty of care to safeguard their patients and people in their care.
